Form IMM 5669, also known as the Schedule A: Background/Declaration Form, is required by Immigration, Refugees, and Citizenship Canada (IRCC) during the permanent residency application process. It is used to collect detailed personal history information to assess an applicant's eligibility and conduct necessary background checks.

The Principal Applicant, their spouse or common-law partner, and any dependent children aged 18 or older applying for permanent residency must complete this form.

It is used to disclose personal history and verify eligibility for immigration.

The Principal Applicant and all family members aged 18 or older applying for permanent residency.

It is not required for family members of refugee claimants who are not in Canada.

Submit it as part of your permanent residency application package to IRCC.
